I agree that Sarah Sonnenschein dba Sarah’s Dance Studio along with employees shall not be liable for any injuries, damages, or losses sustained by me or my child/children which are in any way related to their participation in activities sponsored by Sarah’s Dance Studio. I assume risks and accept full responsibility for any injuries, damages, or losses my child/children may incur in connection with my decision to exercise my permission to participate in Sarah’s Dance Studio. I agree to release Sarah’s Dance Studio from any liability due to my child’s/children’s participation in the program. As a parent or legal guardian on the student’s listed above, I understand the waiver and release and am in full acceptance of the conditions written therein. I hereby state that I am freely allowing my child/children listed above to participate in Sarah’s Dance Studio.

Tuition is due by Quarter and is due on the first day of October, December, February and April in the form of cash or check (payable to Sarah’s Dance Studio). Tuition is considered late if not received by the 2nd week of the quarter, in which a $10.00 late fee is applied. A $20.00 fee will be applied for all returned checks.

There are 4 dance quarters per year.

Please understand that once a quarter begins, refunds and transfers are not permitted.
